What Is Changing With Medicare Drug Plans in 2027?

Each fall, Medicare sets new limits for Part D prescription drug plans. Here are the official 2027 numbers every Tampa Bay senior should know before choosing a plan.

$2,400

Yearly Out-of-Pocket Cap

The most you pay in 2027 for covered drugs before your plan pays the rest of the year. It was $2,100 in 2026.

$700

Maximum Deductible

The highest deductible a drug plan may charge in 2027, up from $615. Many plans charge less, and some charge none.

$35

Monthly Insulin Cap

Covered insulin stays capped at $35 a month per product, a protection that continues into 2027.

25%

Coinsurance Rate

After your deductible, you typically pay 25% of drug costs until you reach the $2,400 cap.

Figures confirmed from Medicare.gov and CMS announcements for the 2027 plan year.

How a Medicare Prescription Drug Plan Works

A Part D drug plan shares the cost of your prescriptions with you in three simple stages. Once you know the stages, plan comparisons make a lot more sense.

1

Deductible Stage

You pay the full price of your drugs until you meet your plan's deductible. In 2027, no plan may charge more than $700, and some plans have no deductible at all.

2

Initial Coverage Stage

After your deductible, you pay 25% of the cost of covered generic and brand-name drugs, and your plan pays the other 75%.

3

Catastrophic Stage

When your 2027 out-of-pocket spending on covered drugs reaches $2,400, you pay $0 for covered drugs for the rest of the year.

What counts toward the $2,400 cap: deductibles, copays, and coinsurance for drugs covered by your Part D plan. Your monthly premium does not count, and drugs not on your plan's formulary generally do not count either.

Your Options for Medicare Drug Coverage in 2027

There is more than one way to get prescription coverage with Medicare. Here are the paths we can review together.

Stand-Alone Prescription Drug Plan (PDP)

This is a separate drug plan you add to Original Medicare. It pairs well with a Medicare Supplement (Medigap) plan, since Medigap does not cover prescriptions.

Medicare Advantage With Drug Coverage

Many Medicare Advantage plans bundle doctor, hospital, and drug coverage in one plan. We can compare what these plans cover against stand-alone PDPs for your exact medications.

Extra Help (Low-Income Subsidy)

If your income and resources qualify, this Medicare program pays your drug plan premiums and deductibles and lowers your copays. Extra Help is not going away in 2027, and we can check whether you qualify.

Medicare Prescription Payment Plan

This option spreads your out-of-pocket drug costs across the year in monthly payments instead of big pharmacy bills. It continues in 2027, and you must opt in again if you switch plans.

Why 2027 Is a Year to Review Your Drug Plan

A federal program that held down stand-alone drug plan premiums ended after 2026, so some premiums are changing more than usual this year. Here is what that means for you.

Some Premiums Are Rising

With the temporary premium stabilization program over, staying in the same plan without checking could cost you more each month in 2027.

Plans Change Every Fall

Drug lists, copays, and rules change each plan year. The plan that was best in 2026 may not be best for 2027.

Your Medications Matter Most

The right plan depends on the specific drugs you take. We compare plans against your actual medication list, not just the premium price.

2027 Medicare Drug Plan FAQ

What is a Medicare Prescription Drug Plan (PDP)?
A PDP is a stand-alone insurance plan that covers prescription drugs. You add it to Original Medicare, and it works alongside a Medicare Supplement plan. Medicare Part A and B on their own do not cover most prescriptions, so a drug plan is how most people get pharmacy coverage.
How much will I pay out of pocket for drugs in 2027?
It depends on your plan and your medications, but Medicare sets the limits. In 2027, the most you can pay out of pocket for covered drugs is $2,400 for the full year. After that, your covered drugs cost you $0 for the rest of the year. No plan may charge a deductible higher than $700, and after your deductible you typically pay 25% of drug costs until you reach the cap.
Is insulin still capped at $35 a month?
Yes. Covered insulin products remain capped at $35 for a month's supply per product under Medicare drug plans in 2027.
Why might my drug plan premium go up in 2027?
A temporary federal program that helped hold down stand-alone drug plan premiums ended after 2026. Because of that, some stand-alone plans have larger premium changes than in recent years. This is exactly why we recommend a yearly review of your plan, your drugs, and your total costs.
What is Extra Help and do I qualify?
Extra Help is Medicare's Low-Income Subsidy program. It pays your drug plan premiums and deductibles and lowers your copays. It is based on income and resources, and it is not changing in 2027. If you think you might qualify, ask us and we will help you check.
When can I change my drug plan for 2027?
The Medicare Open Enrollment Period runs October 15 through December 7, 2026 for coverage starting January 1, 2027. If you have a Medicare Advantage plan, you get a second window from January 1 through March 31, 2027.
Do you charge for your help?
No. Our help is free to you. If you enroll in a plan through us, the insurance company pays us a commission, and your plan price is exactly the same either way. There is never any pressure or obligation.
Do you really come to my house?
Yes. In Pasco, Pinellas, Hillsborough, Polk, Citrus, Hernando, Manatee, and Sumter counties, we schedule in-person appointments at your home at no charge. If you live outside those counties, we are happy to help you by phone.
What should I have ready for our appointment?
Have your medication list or pill bottles, your red-white-and-blue Medicare card, and any letter from your current plan about 2027 changes (often called an Annual Notice of Change). That is all we need to compare your options.
What if I just have a quick question?
